They are advised to forward the following forms duly completed, by Speed Post/ Courier immediately and in any case not later than five days from the date of this E-mail to the Board’s address mentioned below.

1. Five copies of the Attestation Forms (all in original) attested by an authorized official stated therein

2. Six copies of Bio-data forms

3. One set of the certificates (Photocopies) in support of your date of birth, educational qualifications, experience, caste (SC/ST certificate /OBC certificate),  OBC declaration form  etc.) and PWD status.
