Please refer to our letter DGBA.CO.DT.No.13.01.299/H.6252/2003-04 dated April 22, 2004 on the captioned subject. In this context we advise that you may take necessary steps for using the revised application forms in all the designated branches of your bank with effect from June 15, 2004.
